All clients are required to provide and maintain a valid credit or debit card on file before any appointment is scheduled. The Payment Authorization form is required and must be signed before the first appointment.
Full payment is due at the time of service. If insurance benefits apply, estimated copays and deductibles are due at the time of service.
Mr. Baker accepts cash, checks, credit, or debit cards.
Making an appointment with Mr. Baker is a verbal arrangement for his time. Unfortunately, when a client does not honor their scheduled appointment, it costs our office valuable time that could be spent helping other clients. This is a small business, and to prevent loss of time and money due to client no-shows or last-minute cancellations, a fee will be charged for any no-shows or late cancellations. A late cancellation is less than 48 business hours. No-show and late cancellation fees are billed at $150 per hour of the scheduled appointment.
Mr. Baker currently accepts Blue Cross and Blue Shield.
Please remember your insurance policy is a contract between the client and their insurance company. As a courtesy, our office provides certain services, including verification of in-network benefits and a pre-treatment estimate. The full allowable amount insurance lists for us is due at the time of service. For all other insurances, the private pay rate is required in full.
Please be aware that filing for services is not a guarantee of payment. The services provided may or may not be covered by insurance. Regardless of any insurance status, any balance is the client's responsibility. The client is responsible for all professional services rendered.
The private pay rate for a comprehensive psychological evaluation with Mr. Baker is $1,700. This includes an intake appointment, two testing appointments, and a results appointment (please refer to "Services" for a detailed description of each appointment). This fee can be paid in full at the intake appointment or paid in installments at each appointment. The breakdown of the total cost is as follows:
1-Hour Intake Appointment: $150
Two 3-Hour Testing Appointments: $1,400 ($700 per appointment)
1-Hour Follow-Up Appointment: $150
Our office can provide you with a super bill that itemizes the charges and includes the billing codes for each appointment. You can submit the super bill to your insurance to try to seek reimbursement as an out-of-network provider.
